Q: Palliative care is deeply personal work. What's the philosophy driving Tuesday Health?
A: Tuesday Health launched in 2023 to deliver compassionate, supportive care for patients and caregivers navigating serious illness. We believe that each patient's journey is unique. As such, we deliver member-directed, whole-person care to seriously ill patients in our program, using leading-edge supportive care models, including appropriate transitions to hospice when the time is right.
This results in dramatically improved quality of life for members and their caregivers, and a meaningful reduction in unproductive medical spend. Our overall mission is to transform serious illness and end-of-life care; our team believes deeply in this mission and puts our members first in all that we do.
Q: Value-based palliative care depends on understanding patient risk. What was getting in the way?
A: Primarily limitations in our claims data. We get claims extracts from our clients, but they are slow and have limited detail. Our success in value-based care is predicated on our ability to understand patient risk and determine the appropriate frequency and intensity of services. We needed a way to get a more comprehensive picture of the patient in a more real-time way.
Q: What was your clinical workflow like before you had access to comprehensive patient records in Metriport?
A: Very lengthy intake assessments and limited summary information from claims. Many of our patients are seriously ill and often unable to give a complete or accurate medical history on their own. Even for patients who can share details, having access to their prior records allows our clinicians to focus their limited time on decision-making and patient support, rather than trying to reconstruct a patient's history during precious visit time.
Q: Walk us through how Metriport fits into a typical day for your clinical and data teams.
Donald Rauscher, Head of Data & Analytics: When we schedule patients for visits, we perform document queries, pull back those PDF summaries, and attach them to patient records in our care management system. We're analyzing encounter and discharge notes from inpatient stays, leveraging AI to automate root cause analysis, and using those results to automate patient phenotyping.

I also use Metriport before our weekly clinical rounds, where we review high-risk patients who have either been hospitalized recently or may be eligible for hospice. Having this consolidated information readily available makes our team discussions more efficient and grounded in current data.

Emily: The AI Summaries have been incredibly valuable for me and my colleagues. They provide a concise, at-a-glance overview that makes a real difference in fast-moving clinical situations.
For example, if I'm pulled into a patient visit without time for a full chart review, the summaries allow me to feel prepared and confident in supporting our nurses in the field. I particularly appreciate how the summaries are tailored with our needs in mind—highlighting recent inpatient events, serious illness diagnoses, hospitalizations, as well as code status and goals of care discussions.
Q: You're a growing startup. How has Metriport shown up as a partner?
Donald: Very responsive! Tremendously helpful in helping us build out our integration on the API and data platform side, and also incorporate directly into our care management system.

Q: Since data is so critical for value-based care, how has Metriport changed the equation?
Donald: Our success is predicated on our ability to understand patient risk and determine the appropriate frequency and intensity of services. Metriport gives us a much more comprehensive picture of the patient in a more real-time way than claims data alone ever could.
We looked at ways to incorporate Metriport data into our homegrown patient risk stratification tooling, which we call PULSE. This integration helps us better identify which patients need more intensive support and when.
What we found is that our PULSE model performance improved with the inclusion of features from the Metriport data, principally lab results data. Coverage by our top segment increase by 12% for our acuity model and 22% for our prognosis model! This model really helps us align our clinical resources with those with greatest need and advance our VBC performance.
Leo: We're also excited to explore using Metriport's chronic condition and medication data to help seed patient profiles in our platform, Elevate. This will save valuable time for our nurses and reduce the need for manual data entry during visit preparation. We're also looking forward to building deeper integrations, such as linking into Metriport directly from our platform, to further streamline access to external records and improve efficiency.
Q: You're also contributing data back to the network. What does that look like?
Donald: We share notes from our encounters, which include documentation on patients' advanced directives and key assessment results.
Q: What's next for Tuesday Health, and how does Metriport fit into that future?
Donald: Growth! In addition to expanding with our existing customer, we're going live with two additional plans in 2026, which will involve scaling our team and our systems to serve more patients across multiple geographies.
